The Delivery System Transformation team has roots in designing and facilitating policy change, and in leveraging policy to help create broad public commitment to high-quality care. As a Long-Term Care Project Manager, the successful candidate will have expertise in aging and disability to support multiple projects. Projects range from supporting our state partners with administrative support, policy design, implementation and re-design and improving access to and resources for individuals experiencing a disability through resource development, training, and policy recommendations. Altarum’s Delivery System Transformation team also engages clinical providers in improving care delivery through direct training and technical assistance in optimizing the use of electronic health records, data sharing with community-based organizations, enhancing team workflows, and better understanding evidence-based practices.
 
This position requires working effectively with diverse clients, management, contractors, and consultants to successfully manage large and dynamic projects, with little supervision.  The successful candidate will have the opportunity to use their experience and expertise to generate insights that inform equitable public health and health services solutions. While at Altarum, the Project Manager will work on and across many projects with specific tasks and responsibilities varied across projects.
 
In addition, we’re looking for a leader who has experience mentoring staff, building team morale, and engagement. The ideal candidate has demonstrated the ability to empower and motivate team members to bring their “best self” to their project work, supervising project teams to achieve project goals and objectives, managing timely completion of all project deliverables within budget and defined timelines, and supporting project activities, as appropriate. To be successful in this role, the candidate must have a robust attention to detail and an ability to maintain a positive relationship with clients and teammates.
