Community Health Worker – NorthShore
Position Highlights:
Position: Community Health Worker (CHW)
Location: Evanston, IL
Full Time/Part Time: Full Time
Hours: Monday-Friday, day shift. In addition, there may be occasional evening or weekend hours for community events.
Travel: Frequent local travel will be required. You must either maintain a valid State of Illinois Driver’s License with proof of active insurance or have access to other means of transportation.
What you will do:
Assess and address patient’s needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient’s health outcomes (Social Determinants of Health).
In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment.
Assist patients with getting access to social services and other community-based resources.
Follows up with patients via phone calls, home visits and visits to other settings where patients can meet.
Maintain accurate documentation of patient contacts and document patient’s needs, action plan and outcomes in the medical record system.
Represents the organization at various community events.
Participate in continuing education, trainings and CHW organizations.
What you will need:
Education: High school diploma or GED
Experience: Minimum of 2 years of community based experience.
Skills:
Fluency in Spanish is strongly preferred
Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations.
Ability to effectively engage in conversations, motivate and encourage patients to take steps to maintain and improve their health.
Creative thinker with ability to problem solve and brainstorm solutions.
Ability to work collaboratively as a part of a patient care team.
Ability to work independently with minimal supervision.
Leadership and community education experience.
Basic computer skills required. Experience with EPIC EMR preferred.
Fluency in Russian, Urdu, Hindi, or Arabic a plus.
Certification: Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred.
