Location: University of Oklahoma Medical Center
Shift: Full-Time (40 hours/week) 8-hr shifts

POSITION INFORMATION: Provides professional nursing-related healthcare in a medical specialty. Assesses, plans, implements, and evaluates nursing care in a designated patient population or specialty. The RN Coordinator manages, oversees and coordinates the care and services of patients and their families who have been referred to the Palliative Care Team.
REPRESENTATIVE DUTIES:
Clinical responsibilities:
Intake of new inpatient consults
Schedule inpatient “appointments”
Coordinate care conferences
Management of comfort cart
Hospice referrals and enrollment
Coordinate/complete discharge follow up calls
Attend tele-health visits (once monthly)
Facilitate transitions from perinatal palliative care to general palliative care service
Facilitate transitions from OCH to BCHC and vice versa
Facilitate enrollment into bereavement program
Triage of oncology consults
Intake of oncology patients
Coordination of oncology clinic schedule with palliative care visits
Education and Quality:
Graduate from an accredited school school of nursing required.
Develop and maintain educational materials for patients/families
Manage/maintain palliative care patient database
Quality improvement initiatives
Equivalent/ Substitution:
Will accept an Associate’s degree in Nursing plus an additional 24 months of experience in lieu of the Bachelor’s degree in Nursing
Experience:
12 months related experience in the designated specialty
Pediatric nursing experience, ideally in an acute care setting or in a setting with patients who have chronic, complex needs
Palliative care or hospice experience can be considering in lieu of pediatric experience
License(s)/Certification(s)/Registration(s) Required: Current RN License issued by the Oklahoma State Board of Nursing, or a current multistate compact RN License (eNLC). Current Basic Life Support issued by the American Heart Association required upon hire.
