Essential Duties:

  • Develops specific, measurable, realistic plans and objectives, which enhance social service delivery.
  • Demonstrates ability to perform a psychosocial assessment and reassessment on all patients.  This includes pediatric, adolescent, geriatric and the general patient population.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Communicates with outpatient physician regarding social service needs of the patient.
  • Maintains current knowledge of resources available within the community. Maintains supply of resource materials to be distributed to patients when needed.  Is able to obtain other resources as needed.
  • Communicates with family members and caretakers regarding the needs of the patient and anticipated plans as appropriate.
  • Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Family is included in teaching as appropriate.
  • Treats patients and their families with respect and dignity.  Identifies and addresses psychosocial needs of patients and family.
  • Manages and operates equipment safely and correctly.
  • Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
  • Communicates appropriately and clearly to physicians, staff and administrative team.
  • Accurately determines type of assistance/setting/resources necessary for the patient/family and provides appropriate resources/assistance/list of facilities.
  • Demonstrates an ability to be flexible, organized and function under stressful situations.
  • Maintains a good working relationship with the outpatient clinic staff and the community.
  • Interacts with all individuals in a consistent manner, providing attention, support, and assistance to foster an environment of exceptional personal service.
  • Maintains regulatory requirements.
  • Complies with all organizational policies regarding ethical business practices.
  • Maintains confidentiality of information at all times.
  • Maintains a safe work environment and performs duties of the position in a manner consistent with ensuring the safety of self and others.
  • Contributes to a positive learning and teaching environment for students and residents by participating formal educational activities as requested, providing feedback in the context of clinical care, and being open for questions and consultation

Education and Experience:

  • Masters in Social Work and
  • Licensed in the state of Maine.
  • 3 or more years of previous experience in all aspects of social services.

Knowledge, Skills and Abilities:

  • Additional languages preferred.
  • Ability to engage patients and team members utilizing the CMH Experience Standards
