Position: Clinical Coordinator

Schedule: Monday through Friday 8:00a-5:00p

In this role, you will help support the field staff in the office and may go into the field for emergent needs.

Key Responsibilities:

  • Team Building: Assemble and lead teams dedicated to delivering the highest quality of life to our patients.

  • System Management: Maintain organized and efficient systems to ensure the continuity and quality of patient care.

  • Operations Evaluation: Monitor agency operations, track key metrics, and identify opportunities to enhance workflow and productivity.

  • Medical Record Support: Assist staff in maintaining accurate medical records and utilization reviews.

  • Non-Clinical Support: Aid the clinical supervisor by handling non-clinical phone inquiries.

  • Problem Reporting: Notify supervisors of any issues requiring administrative attention or intervention.

  • Administrative Duties: Manage daily mail, inventory, and equipment cleaning for the department.

  • Hospitalization Prevention: Implement strategies to prevent avoidable hospitalizations and ER visits.

  • Ongoing Learning: Attend training, education seminars, and other learning opportunities to stay informed.

  • Patient & Caregiver Interaction: Provide the highest level of care and support to patients and caregivers.

  • Communication: Maintain positive and supportive communication with physicians, patients, families, and agency personnel.

  • Mission Promotion: Uphold and promote Elara Caring’s philosophy, mission statement, and administrative policies.

  • Privacy Compliance: Ensure patient and staff privacy and confidentiality per HIPAA regulations.

  • Additional Duties: Perform other duties and projects as assigned.

Requirements:

  • Current LPN License: Must meet state requirements.

  • Experience: At least 1 year in acute care or an equivalent setting.

  • CPR Certification: American Heart Association or American Red Cross.

  • Positive Attitude: Bring energy and enthusiasm to every patient interaction.

  • Dedication to Quality Care: Commitment to providing the best patient care possible.

  • Strong Communication Skills: Ability to connect with patients, families, and team members.

  • Reliable Transportation: Essential for performing job duties.
