Job Title:                APRN/Nurse Practitioner

Department:             Hospital Medicine

Job Summary:   We are looking for a skilled and flexible On-Call Nurse Practitioner (NP) to provide high-quality, responsive care in hospitals and Skilled Nursing Facilities (SNFs). This role requires a dedicated professional who can effectively manage patient needs during critical times, ensuring continuity of care and support across multiple settings.

Key Responsibilities:

  • Direct Patient Care
    • Provide on-call primary and acute care to patients in hospitals and SNFs as needed.
    • Conduct thorough patient assessments, including physical examinations, history taking, and diagnostic evaluations to formulate appropriate treatment plans.
  • Care Coordination
    • Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure seamless patient care and effective communication regarding patient status and treatment plans.
    • Serve as a clinical resource for nursing staff, providing guidance on complex cases and urgent care situations.
    • Manage transitions of care for patients moving between the hospital and SNF settings, ensuring continuity of treatment and follow-up.
    • Coordinate care with primary care providers and specialists to optimize patient outcomes.
  • Quality Improvement & Data Management
    • Participate in quality improvement initiatives aimed at enhancing patient care and reducing hospital readmissions.
    • Maintain accurate and timely documentation of patient encounters, ensuring compliance with regulatory standards and facility policies.
  • Education & Mentorship
    • Provide education to nursing staff and clinical personnel on best practices, protocols, and policies relevant to patient care in acute and long-term settings.
  • Documentation & Compliance
    • Ensure accurate and timely documentation in electronic health records (EHR) in accordance with facility policies and regulatory standards.
    • Maintain compliance with relevant regulations and quality standards.
  • Availability & Responsiveness
    • Respond to on-call requests promptly and provide care in a timely manner, ensuring adequate coverage during peak times or staff shortages.
    • Maintain flexibility in scheduling to meet the needs of the facility and its patients.

 Job Qualifications:

  • Education: Master’s degree in nursing from an accredited program.
  • Certification: Current Nurse Practitioner certification and state licensure to practice as a Nurse Practitioner.
  • Experience: Previous experience in a Skilled Nursing Facility, geriatrics, or related field preferred.
  • Skills: Strong clinical assessment and diagnostic skills, excellent communication and interpersonal abilities, and proficiency in electronic health records (EHR).
  • Attributes: Compassionate, team-oriented, adaptable, and committed to delivering high-quality patient care. Willing and open to accepting feedback.

Work Environment: This position typically involves working in a HIPAA compliant, with secure access to Wi-Fi to ensure the protection of patient information and maintain confidentiality during communications and documentation.
