As a RN Clinical Field Staff Liaison (CFSL), the work you do every day makes a difference in the lives of our patients by ensuring quality and safe delivery of home health services.

HOW YOU'LL WORK:
You’ll provide support and act as a resource to clinicians in the field. You will assist in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management.

MAJOR AREAS OF RESPONSIBILITY: 

  • Operations: Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns.  Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to manager.  Monitor Home Health Aide supervisory visits.

  • Patient Care: Work in the field with staff and patients as needed.

  • Customer Service: Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families.

  • Minimize Patient Risk: Ensure Start of Care compliance and quality and safe delivery of home health services. Focus on Outcome Monitors as assigned.  Implement quality improvement programs in the field.

  • Policies: Review clinical documentation and maintain current knowledge of all Medicare/ Federal/ Insurance / Company and Hospital guidelines, policies and protocol. 

  • Teamwork: Assist with monthly Case Manager, Nurse and Therapy meetings.  Participate in all on-call requirements, and patient staffing as needed.

  • Attention to detail is critical, as is being observant and following directions.

  • Critical thinker and the ability to problem solve clinical needs.

 
REQUIREMENTS:

Minimum one-year home health experience is preferred

  • Current RN license in the state of employment

  • Minimum of Three (3) years of experience as an RN in an acute care setting 

  • Participates in on-call, evening/weekend shifts and provides patient care, per agency needs

  • Valid driver's license and auto insurance in your name as a driver  

  • Capable of all physical demands
