Hospital in the Home Nurse - Grade 3B Registered Nurse 

We are seeking dynamic Registered Nurses to join our rapidly expanding, multi-disciplinary Hospital in the Home (HITH) team.  

The HITH nurses will work collaboratively with our HITH Medical, and Allied Health staff to facilitate efficient and holistic home-based care for our diverse community. 

As the successful applicant, you will become part of a growing multidisciplinary team at a critical time in the history of St Vincent’s Hospital Melbourne, as we transform traditional inpatient care towards a ‘home first’ approach. 
 
About the Role 


The HITH Nurse will provide high quality nursing services including assessment of broader care needs to a culturally diverse community. The HITH Nurse is responsible for improving the health status and independence of the consumer and supporting their carer/s in their role, working as part of a team to balance workload, and new patient referrals on a daily basis. 

Classifications for this position will range from YU15 ($102,278.80 per annum pro rata)

 

Duties 

  • Provide high quality nursing services delivered within professional boundaries and responsibilities including assessment, care planning, treatment, review and education of eligible patients and their carer/s.

  • Provide on call service to HITH patients as rostered

  • Ensure an effective transition to HITH, and between services, that reflects the needs of the patient

  • Provide services which are accessible, responsive and support the needs of diverse communities 

Qualifications, Skills and Experience 

  • Minimum of 3-4 years post-graduate experience

  • Relevant experience within the community setting and/or across a broad range of acute medical/surgical services

  • Demonstrated advanced clinical skills and knowledge of best practice in clinical nursing practice

  • Demonstrated highly effective interpersonal skills including problem solving, conflict resolution, negotiation, and teaching.

  • Experience working with clients with multiple comorbidities and complex care needs that impact safety, function and independence.
