Serve as a liaison between patient and physician for information related to Maternal-Fetal Medicine coordinating inpatient and outpatient care for patients.
• Collaborate with all members of the Maternal Fetal Committee, medical staff, and administration to provide quality comprehensive care to patients.
• Maintain patient case log with navigation notes and progress through continuum of care.
• Identify or assist in developing appropriate education material with input and approval of the Nursing Education Department.
• Provide and document initial and ongoing education to patients and caregivers about disease, treatment, diagnostic procedures, and other information.

Must exhibit excellent communication skills because this position requires frequent verbal and written communications with physicians, patients, visitors, departmental staff, and affiliated offices. The employee obtains, presents, and discusses clinical information regarding job responsibilities on a routine basis. Candidate must be able to explain or give directions to maintain goodwill and obtain cooperation with all contacts. Must possess the ability to function independently and under pressure while still actively participating in a team environment. Position requires the employee to always exhibit excellent customer service skills.
Must possess the ability to function independently, yet productively as a cooperative team member with various work teams associated with the position. Must be compliant, with a willingness to follow all Hospital policies as well as the established practices, protocols and procedures of the position, department, and applicable professional standards.
Must be willing to support patients in need of assistance for physical, psychological, and social needs with one-on-one contact from any point in the continuum of care from notice of pregnancy risks and/or treatment and recovery. Assesses patient needs and provides educational resources to patient and family answering questions and addressing concerns during the pregnancy. Facilitates physician recommendations for patient care to ensure patient compliance and understanding. Focuses on achieving outcomes within an effective and realistic time frame.
Maintains patient case log with navigation notes and progress through continuum of care. Documents the assistance and monitoring of patients who are “navigated” through the health care system monitoring efficiencies and effectiveness of providing education and needed services across all product and service lines to ensure proper access to care, guidance and support throughout the process.
Must be able to independently perform job functions, as well as be able to work within the organization and through external services to identify and eliminate barriers to health care to ensure availability of services and accessibility of care services. Assists in increasing access to culturally appropriate, supportive care when and where possible.
Must be able to ensure that quality, confidentiality, and professionalism are threaded throughout all aspects of care and programming. Inherent in health patient navigation is continuous quality care for patients from presentation of risk in pregnancy.
Must be dependable, accountable, cooperative, and flexible with work hours to meet department needs, and provide an open climate to promote communication with patients, family members, community healthcare providers and NCH staff and providers. Provides current and appropriate pregnancy information as requested by patients, family members and staff.
In collaboration with the OB/GYN and pediatrics divisions, the Nurse Navigator evaluates patient outcomes. Assures forms, policies, procedures are current in assigned areas and keeps the team informed of clinical and operational issues.
Must be able to respond to all requests for consultation in a timely manner – Completes assignments in a reasonable amount of time and shares, researches and interprets the latest information on changes in health and care to heal keep staff, health care providers, patients and family members up-to-date. Remains knowledgeable about women’s health and pregnancy related risks.
Assist the leadership team to identify methods to improve the financial status of the maternal fetal medicine program. Evaluates and changes programs and/or services of the patient navigation program in response to changing patient population needs, new technology and organizational goals. Evaluates the cost-effectiveness of the maternal fetal medicine patient navigation program on an on-going basis.
