JOB DESCRIPTION:

Provides leadership and supervision for the Supported Housing program. Serves as a liaison between Centerstone and assigned contracted supported housing providers. Ensures coordination of care is occurring. Provides education and consultation to staff. Manages workflows to ensure proper support and resources are available for clients and staff.

Supports the Centerstone Mission, Vision and Values in delivering care that changes people’s lives. Communicates clearly and appropriately; provides excellent customer service. Adheres to applicable policies and procedures. Regular attendance and punctuality is required.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Serves as care coordinator.
  • Work with clients and staff to develop service plans.
  • Work with clients and staff to complete and update continued stay reviews and submit continued stay reviews on-time.
  • Coordination of admissions and transfers.
  • Complete auditing of Supported Housing facilities.
  • Assist providers with corrective action when needed.
  • Provide psychosocial training, education and consultation to staff.
  • Provide client advocacy.
  • Assist with behavioral planning as needed.
  • Provide oversight with discharge planning.
  • Submit adverse incident reports as needed.
  • Monitor TennCare eligibility.
  • Assists with “onboarding” of new sub-contracted Supported Housing facilities.
  • Attend required meetings including clinical supervision.
  • Relays pertinent information to supervisors in a timely manner.
  • Provide coverage as needed for fellow Housing Specialists.
  • Other reasonable duties assigned.

MINIMUM QUALIFICATIONS

  • High School
  • One (1) year of related experience
