About the role:
The Specialist Pharmacist Heart and Lung Transplant, is a specialist clinical role. The purpose of the role is to provide comprehensive clinical pharmacy services across St Vincent’s Hospital and affiliated services for the defined specialty area of Heart and Lung Transplantation.
The Specialist Pharmacist Heart and Lung Transplant will work closely with our Lead Pharmacists in Heart and Lung to support transplant patients through their journey from pre-transplant workup, immediately post-transplant, to ongoing support through the outpatient clinic.
St Vincent’s Hospital provides the only adult Heart and Lung Transplantation service in NSW and is one of the largest and longest running programs in Australia, with survival rates exceeding that of international benchmarks. The unit is a leader in its field, having performed Australia’s first successful heart transplant in 1984, and continues to deliver cutting edge research and innovative care in this field. This is an exciting and unique opportunity to be part of this internationally recognised team.
All Specialist Pharmacist roles are integral members of the SVHS Pharmacy team to provide specialist clinical knowledge and skills for the pharmacy service. The position will work closely with Team Leader and Lead Pharmacist positions, to maintain comprehensive pharmacy service across all operational hours.

Selection Criteria:
1. Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent’s Health Australia
2. Current Pharmacy Registration with AHPRA with no scope on conditions of practice, able to meet the criteria set out in the Health Employees' Pharmacists (State) Award for a Grade 2 Pharmacist, and ability to provide weekend and on-call out of hours service as per roster.
3.Experience and a demonstrated high level of performance, competence, expertise, skill and application of clinical judgment in the provision of pharmacy services to Cardiology, Respiratory, Critical Care and/or Transplant patients including relevant governance and regulatory requirements.
4. Excellent written and oral communication skills as well as demonstrated effective interpersonal skills including the ability to consult with other health professionals, patients and carers.
5. Demonstrated knowledge of Drug and Therapeutics Committee processes including Formulary management and experience in the implementation of formulary decisions and policies and procedures.
6. Demonstrated ability to effectively work in a team and fostering a positive workplace culture
7. Demonstrated skills and experience in managing a complex workload, excellent organisational and time management skills and a high attention to detail and accuracy in undertaking all tasks
8. Demonstrated commitment to professional development and demonstrated experience in research, Quality Use of Medicines and medication safety initiatives.
