Job Description Summary:

To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Actively participates in departmental functions.

Minimum Qualifications:

Bachelor's Degree: Physical Therapy (Required)DL-HC - Driver's License only if in a Home Care department - Department of Motor VehiclesDepartment of Motor Vehicles, LIC - Licensed in Field - State of OhioState of Ohio, PT1 - Physical Therapist License - American Board of Physical Therapy SpecialtiesAmerican Board of Physical Therapy Specialties

Licensed in the State of Ohio. Basic computer skills. Entry level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical Experience

For this position, in the Home Care department only, Minimum Qualifications include: Valid state driver’s license, Minimum of eighteen (18) years old, Have an acceptable Motor Vehicle Report (MVR), Must provide proof of insurance, if applicable.
