MediSystem Pharmacy, a Shoppers Drug Mart Company, is an established specialty pharmacy that provides pharmaceutical dispensing and clinical pharmacy consulting services to long term care facilities across Canada. Our MCMR team is looking to hire a full-time Medication Reconciliation Specialist to join our dedicated team that is currently based out of our Mississauga operations site and serves our long-term-care facilities across all of Ontario. The Medication Reconciliation Specialist will ensure all new admissions and readmissions receive a thorough medication reconciliation before moving into or returning to their homes.  This requires due diligence with information gathering and collaboration amongst different teams and families as well as working closely with our homes.  We offer competitive pay, full benefits, and a work-life balance.

What You'll Do:

  • You will use your strong communication skills to discuss important information between colleagues and external partners (physicians/nurses located at the long-term care home), as it relates to the patient/resident prescriptions and their profiles
  • Provide friendly customer service to foster relationship building with our external partners and clients
  • Excellent organization and time management skills
  • Strong communication skills (both oral and written)
  • Self-motivated with a passion for providing high quality client services and building relationships
  • Strong computer skills with the ability to navigate through a variety of computer systems quickly
  • Previous work experience using Kroll
  • Minimize medication incidents according to company policies and procedures.
  • In the event of a medication incident follow protocol for reporting and resolving.
  • Participate in continuous learning activities and share relevant learnings with other staff 
  • Protect patient privacy and confidentiality in accordance with regulations.
  • Comply with all federal and provincial pharmacy regulations.
  • Comply with all company policies and operating procedures (regulatory/legal requirements) to assist with delivering high-level pharmaceutical care.

  • Strong computer skills with the ability to navigate through a variety of computer systems quickly
  • A high degree of accuracy and attention to detail
  • Superior interpersonal skills, with a commitment to provide exceptional customer service
  • Be highly organized, take initiative, and has excellent problem solving and decision-making skills

To help you stand out, any or all of the following are preferred: 

  • Previous work experience in long-term care either as a PSW or an RPN, or in a long-term care pharmacy setting as a Pharmacy Assistant
  • Previous work experience using Kroll is an asset
