Our MCMR team is looking to hire a full-time Medication Reconciliation Specialist to join our dedicated team that is currently based out of our Mississauga operations site and serves our long-term-care facilities across all of Ontario. The Medication Reconciliation Specialist will ensure all new admissions and readmissions receive a thorough medication reconciliation before moving into or returning to their homes. This requires due diligence with information gathering and collaboration amongst different teams and families as well as working closely with our homes.
