Job Summary:

The Population Health Clinical Coordinator is responsible for reviewing the health records of patients who are attributed to Wellstar Clinical Partners' value based contracts, identifying gaps in care, providing evidence of care gap closure and assisting patients in obtaining the needed care and appointments to satisfy the focused metrics.

Core Responsibilities and Essential Functions:

Review electronic medical record for open gaps. Patient outreach to assist in scheduling routine preventive visits. Determine best method of outreach for each patients circumstance. (phone, Mychart, text, letter, family, etc.) Pend standing orders for specific gaps for upcoming appointments. Review electronic medical record for supporting evidence of gap closure and upload to the appropriate payer portal or to EPIC using standard workflows. Work with EPIC patient registries to identify patients overdue for Hgb A1c and/or Blood pressure and other metrics as identified by leadership. Interact with patients pharmacies to verify availability of ordered medications. Assist Pop Health Pharmacists with medication adherence outreach calls. Collaborate with WCP practice staff to schedule and arrange care gap appointments. Refer patients to Care Management team for additional concerns or SDOH needs. Interact with specific payer portals to download patient lists and gap reports. Additional Key Responsibilities: Communicate regularly with Population Health leadership to update status. Meet leaderships assignments and expected daily work goals. Maintain strong compliance processes to protect patients privacy and personal health information. Maintain timely and accurate documentation in keeping with standard workflows. Participate in annual ACO-MSSP Quality reporting as assigned by leadership.

Required Minimum Education:

  • Associates Nursing or Trade School Diploma/Degree Nursing or Other / Certificate Medical Assisting or Other / Certificate Pharmacy


Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.

  • Cert Medical Assistant or Lic Practical Nurse-Preferred or Cert Pharmacy Tech

Required Minimum Experience:

Minimum 2 years Population Health or
Minimum 2 years Ambulatory Clinical Experience or
Minimum 2 years Quality reporting Preferred
