Patient Care Representative

As the Patient Care Representative, you’ll contribute to our success in the following ways:

  • Conducts phone calls on a daily basis, communicating with prior patients in assigned territory to follow-up on health status and potential changes in condition warranting additional services.
  • Actively works to connect patients with appropriate internal team members based on patient needs and requests, including but not limited to skilled care, personal care, hospice care, ElaraConnect personnel or others as appropriate.
  • Answers all inquiries regarding enterprise services and provides appropriate information.
  • Collaborates with Patient Care Specialists for leads identified in assigned market territory.
  • Collaborates with Patient Care Specialists and service line operators to resolve issues related to Check-In operations, including connection issues with prior patients and overall program utilization.
  • Remains current on virtual customer relations trends and innovations to meet the needs of the customers.
  • Assists in evaluating agency operations and systems and identifies ways to enhance workflow and productivity.
  • Participates in clinical record audits, utilization reviews and Peer Review and Quality Improvement programs.
  • Attends training, education, seminars, or other means of learning.
  • Notifies supervisor of any problems requiring administrative attention and intervention.
  • Processes all data entry information accurately into appropriate company systems.
  • Participates in weekends and/or holidays, as assigned.

What is Required?

  • High School Diploma/GED
  • 1+ year experience in a home health care setting
  • Knowledge of Medical Terminology
  • Proficiency with MS Office Suite (Word, Excel, Outlook)
  • Excellent verbal and written communication skills.

You will report to the Program Supervisor.
