What You’ll Do 

Help innovate and create a national, community-based, outpatient palliative care program that improves access to and timeliness of palliative care for appropriate pediatric patients. 

  • Patient facing care, which will include 
  • Introducing patients and their families to the PATH service 
  • Holding the PATH “pager”, and provision of reactive nursing cares which may include: 
  • Symptom assessments 
  • Medication adherence and efficacy assessments 
  • Anticipatory guidance for complex disease processes (where appropriate) 
  • Assessment of child and family coping, and referral to higher level of support where appropriate (e.g. behavioral health, social work, etc). 
  • Proactive nursing assessments at intermittent schedules, the timing of which will be jointly determined by RN and PATH team medical director. 
  • Maintenance of PATH team patient census and schedule. 
  • Regular interface with Imagine Pediatrics providers to gather information about referrals in advance of initial PATH visits and Disseminating details of recent appointments and PATH team care plans.  
  • Proactive engagement with market teams(during morning huddle, etc) to evangelize PATH services. 
  • Hospice communication and management, which will include: 
  • Maintenance and dissemination of advance care planning documentation (e.g. OOH-DNR, POLST forms, etc) 
  • Participation in quality improvement activities as assigned or indicated. 
  • Support to Imagine Pediatrics providers, which will include: 
  • Design, implementation, and participation in education and training related to palliative medicine, symptom management, and advanced communication. 
  • Design, implementation, and participation in programs which facilitate provider resilience.  

 
What You Bring & How You Qualify 
First and foremost, you’re passionate and committed to reimagining pediatric health care and creating a world where every child with complex medical conditions gets the care and support they deserve. 
 

  • Graduate of an accredited School of Nursing for Registered Nurse. BSN preferred. 
  • Demonstrated experience working with children and families in a clinical capacity. 
  • Demonstrated experience working with life-threatening conditions, chronic disease, hospice and/or end-of-life care, and related situations. 
  • Demonstrated experience working with family systems. 
  • Willingness to be flexible to an ever changing and growing service 
  • Ability to think critically about program development, and a willingness to be a leader in development of an innovative care model
