Position Summary
The Department of Orthopedic Surgery is looking for an engaged candidate that will build partnerships with patients and the community. Working toward addressing social determinants of health to promote adherence to prescribed treatment plans.
The ideal candidate will provide assistance within Complex Nerve/Hand division of the Department of Orthopedic Surgery, providing guidance to patients and their families as they navigate through community based support services. Meeting with patients face to face or via telehealth based on referral requests from Providers, Advanced Practice Providers, or Clinical Team Members.
Job Description
Primary Duties & Responsibilities:
Assisting Patients with Medicaid Enrollment.
Reaching out to patients regarding repeat late appointment cancellations, continuous rescheduled visits, or repeated No-Shows.
Data Analysis Support for Medicaid/Self-Pay Patients.
System Support and Navigation.
Assisting patients with Financial Assistance Support.
Assisting patients with and following up on transportation to appointments.
Promotes/maintains good public relations with patients and families, acting as liaison between other departments.
Facilitates educational opportunities for Faculty, Advanced Practice Providers, and staff regarding community resources.
Provides appropriate coaching to patients/families to ensure continuity of care and promote health care maintenance.
Acts as an advocate for the rights of culturally diverse patients and their families.
Acts as a liaison to community groups and provides outreach services.
Performs other duties as assigned.

Required Qualifications
Education:
High school diploma or equivalent high school certification.
Certifications:
Advanced Cardiovascular Life Support (ACLS) - American Heart Association, Basic Life Support - American Heart Association, Basic Life Support - American Red Cross, Community Health Worker - Missouri Credentialing Board Inc, Community Health Worker within 6 months - Missouri Credentialing Board Inc

Salary Range
$17.34 - $25.40 / Hourly
The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
