Responsibilities:

Assessment:

  • Evaluate individuals with speech, language, voice, and swallowing disorders.
  • Assess residents' abilities to perform daily living and work activities.
  • Evaluate residents' nutritional needs and dietary habits.

Diagnosis:

  • Identify the nature and severity of communication and swallowing difficulties.

Treatment Planning:

  • Develop and implement therapy plans to improve communication and swallowing skills.
  • Create individualized plans to enhance residents' daily living skills and independence.
  • Develop personalized nutrition plans to support health and wellness.

Intervention:

  • Implement therapeutic activities to enhance physical, cognitive, and emotional well-being.
  • Modify environments and tasks to support residents' needs and abilities.

Counseling and Education:

  • Provide guidance and support to patients and families.
  • Offer nutritional counseling and education to residents and their families.

Monitoring and Documentation:

  • Maintain accurate records of assessments, progress, and treatment plans.
  • Keep detailed records of residents' progress and treatment outcomes.
  • Track residents' nutritional status and adjust plans as needed.
  • Document nutritional assessments, plans, and progress.

Collaboration:

  • Work with other healthcare professionals, such as occupational therapists and registered dietitians, to ensure comprehensive care.
  • Coordinate with other healthcare professionals to integrate nutrition into overall care plans.
