NewYork-Presbyterian has a long tradition of bringing hope, health and healing to our communities. NYP is continuously implementing new programs to meet the unique needs of our communities, and is strengthening those that exist. Our programs build upon, and tie together, the Hospital’s inpatient care coordination, Ambulatory Care Network, and community outreach programs. Now, we're seeking a Program Coordinator to help drive this work forward.  

Program Coordinator – HIV/AIDs Health Home Program - Columbia - Grant Funded

The HIV/AIDS Program provides a comprehensive system of care that includes primary medical care and essential support services for people living with HIV who are uninsured or underinsured. The Program works with cities, states, and local community-based organizations to provide HIV care and treatment services to more than half a million people each year. The Program reaches approximately 52% of all people diagnosed with HIV in the United States. Step into this pivotal role, where you will support family-centered, comprehensive care to women, infants, children, and youth living with HIV centered around their social determinants of health (SDOH). As part of your work, you will also collaborate with staff throughout the NYP system, in our youth partner agencies and within other community-based organizations in Washington Heights/Inwood. The Care Coordinator will meet directly with clients and assist with intake into supportive programs, such as Health Homes, provide motivational interviewing and adherence and behavioral counseling as needed. The Care Coordinator will maintain a panel of active clients and work with program staff to ensure that the goals and objectives of the initiative are fully realized by all clients and meaningfully contribute to ensuring to overall program success by sharing their insight, experience, and creative solutions to care engagement impediments with program leadership.

Preferred Criteria

  • Bilingual Spanish/English
  • Master's Degree in a related field

Required Criteria

  • Bachelor's Degree or 3 -5 years of equivalent experience
  • Proficiency in Microsoft Word
  • Strong interpersonal skills, highly organized, and able to work with diverse community groups and multi-task

Please note that all roles require on-site presence (variable by role). Therefore, all employees should live within a commutable distance to NYP.

NYP will not reimburse for travel expenses.

Salary Range:

$58,500-$79,000
