POSITION PURPOSE:
Provides care under a physician’s order to patients in their homes. Responsibilities include assessment of physical, social, economical and biological needs. Development of a written care plan through direct service, case management and coordination of tasks performed by other multi-disciplinary team members. Functions as the case manager for an assigned group of patients.
ESSENTIAL FUNCTIONS:
- Documents care provided, patient response and other required data according to policies and procedures.
- Implements and participates in patient plan of care according to Federal and State regulations for patients with acute and chronic disease processes in the age ranges from 0-100+.
- Provides direct care and supervises LPNs AND CNAs who provide patient care, functioning as the case manager with the physician, ancillaries, patient and payor.
- Instructs patient or patient’s caregiver to independently provide the care needed including dressing changes, Ivs, medication administration, central line care, SQ/IM injections, pain management and disease process.
- Generates all physician orders, plan of treatments, assessments, care plans, medication summaries, nurse’s notes and discharge summaries with a computerized system.
- Maintains patient caseload, communicating changes in caseload with all members of multi-disciplinary team, generating weekly schedules to keep involved parties informed of caseload status.
- Participates in the performance improvement process by identifying opportunities for improvement, and taking actions to improve patient care and services.
- Follows Medicare guidelines.
- Performs comprehensive OASIS assessments.
- Responsible for assisting in the prevention and control of infections, pain management and medication reconciliation.
SPECIALIZED KNOW-HOW & REQUIREMENTS:
- Current Illinois Registered Nurse Licensure required.
- Minimum of two (2) years' experience in home health, public health or general nursing preferred.
- In-depth working knowledge of current concepts, principles and procedures used in-home care preferred.
- In-depth working knowledge of home care and community services preferred.
- Must have current and maintain certification in Basic Life Support.
- Demonstrates the knowledge necessary to provide care appropriate to any age-related needs of the patients served on assigned unit. Demonstrates knowledge of the principles of growth and development appropriate to the patient population served. Able to assess and interpret data about the patient’s status in order to identify each patient’s needs and provide the appropriate care, including age-related care, needed by the pediatric, adolescent and/or geriatric patient group.
