Job Description​

The Opportunity:

The Senior Clinical Intake Coordinator facilitates the home health and hospice referral process by receiving referrals from multiple referral sources within and without the St. Luke's Health System, performs initial evaluation of referral for home health appropriateness under Medicare, Medicaid, and commercial insurance regulations, determines need for additional information from referral sources, communicates agency acceptance or decline of referral to varied referral sources, and processes accepted referrals.

Shift:  Monday – Friday, daytime hours.

The Work:

  • Facilitates and promotes the Tele-Health services for the agency.

  • Provides specialized support for the Patient Accounting and Finance departments.

  • Serves as liaison between the patient and facility/physician and the third-party payer.

  • Prepares and defends level of care and medical necessity for assigned case.

  • Collaborates with physician advisor, payor representative and site case managers to facilitate appropriate level of care decisions and billing status and ensures compliance with the Utilization Review standard and regulations.

  • Ensures compliance with home health face to face requirements and county reporting for hospice.

  • Periodically participates in remote on-call, evenings, and weekends.

  • May require some travel.
