Patient Flow Coordinator

As a Patient Flow Coordinator, you will work collaboratively to optimize patient flow in an effort to maximize enterprise capacity and support all inpatient placement activities for assigned areas and locations.  
 

A caregiver in this position works 10pm-6am.  


A caregiver who excels in this role will:  

  • Actively participate in the maintenance of 24/7 daily systems to maximize patient flow and optimize bed capacity across the enterprise. 

  • Support centralized communications between referring and accepting provider services for patients that will be admitted as a direct admit from home or office. 

  • Manage bed planning section within EPIC to monitor current and future admissions and discharges to determine impact on patient flow, resources and enterprise capacity. 

  • Communicate and collaborate with Access Nurses, Hospital Transfer Flow Coordinators, and Nursing Operations Managers in triage, prioritization and all bed assignment activities associated with accurate patient transfer and placement needs. 

  • Collaborate with nursing and other ancillary departments to maximize day-to-day patient and bed flow management. This includes, but is not limited to, preparing for and leading bed meetings, monitoring, interacting with, and ensuring accuracy of electronic patient placement systems data, providing backup for patient placement discussions and decisions across the enterprise. 

  • Utilize standardized patient placement processes across all areas of responsibility. 

  • Assign beds for all patients according to diagnosis and/or specific needs. 

  • Communicate hospital census information. 

  • Correct reported census errors and act as a resource to ensure accurate census and patient charges. 

  • Follow HIPPA privacy regulations and Patient Bill of Rights and act as a resource for others in the organization. 

  • Oversee emergency and electronic back-up methods and procedures to support patient flow. 

  • Provide feedback to the supervisors related to process delays. 

  • Meet regularly with Transfer Center team members to discuss, problem-solve, and develop mitigation strategies related to improvement of patient flow. 

  • Actively engage in daily implementation of strategic goals as they relate to capacity management, patient flow and department development. 

  • Interpret and support policies, procedures and safety standards to facilitate daily operations. 

  • Develop and promote long-term relationships with referring facilities. 


Minimum qualifications for the ideal future caregiver include:  

  • High school diploma or GED. 

  • Two years of extensive customer service/healthcare experience, such as patient registration or appointment scheduling. 

  • Knowledge of medical terminology.  

  • Strong customer service skills.  


Preferred qualifications for the ideal future caregiver include:  

  • Previous experience in a hospital admission or patient flow setting.  

  • Working knowledge of computerized hospital admission/bed management systems or other related office functions.  

  • Computer experience in Epic and Microsoft Word and Excel. 

  • Typing speed of 35WPM.

Physical Requirements:

  • Requires sitting for long periods of time, multitasking, manual dexterity to operate computers and normal or corrected vision.
  • Requires working under stressful and emergency situations with the ability to maintain composure in any situation in order to provide superior customer service and ensure proper patient safety and placement.
  • Must be able to perform multiple functions simultaneously without supervision and learn new practices promptly.
