POSITION PURPOSE:         

Plans and provides appropriate ongoing education on for inpatients and outpatients.  Also plans and provides ongoing education to physicians, staff and community groups.

ESSENTIAL FUNCTIONS:

  • Works collaboratively with healthcare providers on outpatient referrals and optimizing patient's self management of chronic medical conditions including diet, activity, medications, self-monitoring and labs.
  • Accurately assesses patient education requirements including evaluation of family support and home situation in compliance with ADA standard of practice.
  • Develops and provides appropriate education to patients to meet needs based on diagnosis type, method of treatment and expected outcome based on clinician assessment.
  • Responsible for gaining and maintaining knowledge on diabetes related technologies and use/interpretation of data on patients' home medical devices including insulin pumps, insulin pens, glucometers and continuous blood glucose monitors.
  • Accurately documents assessment and education provided in the patient medical record.
  • Accurately enter charges for patient services provided.
  • Responsible for contributing to the collection, entry, management and submission of data needed to maintain certification in the American Diabetes Association Recognition program.
  • Provides education presentations as requested for both community groups and medical center personnel.
  • Serves on Diabetes Center task force committee and on team/committees as requested.
  • Other duties as applicable per the practitioner's credential.

SPECIALIZED KNOW-HOW & REQUIREMENTS:

  • Registered Dietitian with a minimum of a Bachelor Degree preferred.
  • Current Diabetes Educator certification preferred.
  • Demonstrates the knowledge necessary to provide care appropriate to any age-related needs of the patients served on assigned unit.  Demonstrates knowledge of the principles of growth and development appropriate to the patient population served.  Able to assess and interpret data about the patient’s status in order to identify each patient’s needs and provide the appropriate care, including age-related care, needed by the pediatric, adolescent and/or geriatric patient group.
  • Demonstrates experience in working with patients with chronic disease.

Compensation Range

$65,561.60 - $98,342.40
