Job Description 

Roles and Responsibilities: 

  • Ensure resident care is provided with established nursing standards and principles, physicians’ orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of residents. Assure observance of required techniques in nursing care. 
  • Assist in assessing residents and develop, implement, review and evaluate resident care plans. Monitor residents’ health, notes changes in condition and assess need for referral to doctors; processes and implement physicians’ orders. 
  • Plan, direct, supervise and evaluate the work of nursing and personal care staff assigned to the unit. Orient and train new personnel and may participate in in-service education. Monitor performance and conduct performance appraisal and refers problems to the Director of Care. 
  • Assist in preparing the Facility for RAI MDS. 
  • Take charge of the Facility in the absence of the Director of Care. 
  • Staff replacement in the absence of the care clerk. 
  • Medication administration as required. 
  • Ability to develop, delegate and/or deliver resident centered care. 
  • Accurate resident centered care plan development, reassessment, evaluation and documentation. 
  • Applied knowledge in Infection Control Practices. 
  • Working knowledge and/or experience with Risk Management, Incident Investigation Management, WSIB and Return to Work practices. 
  • Must be able to meet requirements of all on-line training as required. 
  • Demonstrated ability to supervise and lead care teams in a unionized environment
  • ·Demonstrated commitment to work within interdisciplinary team. 
  • Ability to work effectively independently and with other supervisors, and managers, while prioritizing tasks. 
  • Ability to present information clearly and effectively by demonstrating effective verbal and written skills. 
  • Able to conduct and lead meetings and education sessions. 
  • Commitment to actively participate and present in Quality Improvement Initiatives. 
  • Strong organizational, planning and time management skills. 

 

QUALIFICATIONS  

· Current certificate of competence from the College of Nurses of Ontario required. 

· Relevant experience working within a long-term care environment. (2 years or more preferred). 

· Working knowledge of the MOH Regulations, LTC Act and the Resident Bill of Rights. 

· Familiarity with the homes Specialized Programs. 

· Knowledge and experience, successfully meeting the coding requirements, required time frames and deadlines, in RAI/MDS, Point Click Care, MEDelink. 

· Successful completion of AIS training and ability to maintain annually. 

· Comprehensive knowledge, critical thinking and application of nursing and health care practices.
