SUMMARY:

The Home Health Aide (HHA) is an essential member of the home care team who works under the supervision of a nurse within specified geographical territory. The Home Health Aide (HHA) provides compassionate support with activities of daily living and basic personal care needs while ensuring a safe, comfortable living environment. The Home Health Aide (HHA) effectively communicates and documents any changes in patient conditions to the registered nurse or supervisor and maintains positive relationships.

QUALIFICATIONS & ATTRIBUTES:

  • High school diploma or equivalent.

  • State Tested Nurse Aide (STNA) or 75-hour nurse aide training (CNA). 

  • One year of experience as a nurse aide in a hospital, nursing home, home care agency, or a private setting under the direct supervision of a RN, which can be verified.

  • Completion of skills competency test with 80% or higher score. 

  • Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers.

  • Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients.

  • Valid driver’s license and auto insurance with your name as a listed driver. 

MAJOR AREAS OF RESPONSIBILITY:

  • Performs personal care including but not limited to: bathing, grooming, skin care, toileting, and oral hygiene

  • Light housekeeping after completion of personal care, as applicable

  • Preparation of meals and assists with laundry as needed

  • Assists patients as needed with lifting and transferring

  • Performs simple exercises as an extension of therapy services such as: ambulation and range of motion

  • Maintains good relationships with the patient, the patient’s family, and staff

  • Ensures the living environment is healthy and safe

  • Reports any changes to the patient’s medical and living conditions to the supervisor or the case manager

ESSENTIAL FUNCTIONS:

  • Drives to patient’s primary location per scheduled visit.

  • Capable of all physical demands of lifting, transferring and bathing a physically dependent individual.

  • Daily attendance at assigned visit locations.

  • Documents all aspects of subsequent care visits within 24 hours of visit. Completes and submits all required documentation within specified company requirements.

  • Other duties as assigned.
