Shady Grove Medical Center seeks to hire an experienced Care Navigator who will embrace our mission to extend God’s care through the ministry of physical, mental and spiritual healing.

As a Care Navigator of Behavioral Health you will:
• Identify high and rising risk behavioral health patients who receive care at Adventist HealthCare (e.g., visited the ED or had an observation or inpatient stay).
• Assess, plan, and coordinate healthcare services to meet the patient's specific health care and psychosocial needs in a cost-effective manner.
• Assess need for home visit at initiation of care plan. Home visits may include but are not limited to, focus on medication reconciliation, disease education for patient and family, home safety checks, in-home discussions about care needs and challenges, social needs assessment, and preparation for follow-up appointments with provider.
• Connect patient to community provider(s) and resources for behavioral health care.
• Ensure weekly contact with patient (in person or via telephone) to progress towards care plan goals
• Ensure timely documentation regarding patient enrollment in Behavioral Health Care Management program, patient progress, and graduation from program.

Qualifications Include:
• Bachelor of Social Work, Master of Social Work, Master of Clinical Mental Health Counseling, or other related field
• 2 years of healthcare experience (behavioral health experience preferred)
• Active Maryland Social Work license
• Active Maryland Counselor (LGPC, LCPC)
• American Heart Association BLS Required
• Licensed driver, able to operate a motor vehicle, and maintain automobile insurance

Work Schedule:

Monday-Friday 8:00am-4:30pm

Pay Range:

$63,510.88 - $92,090.78

In cases where the range is displayed as a $0 amount or if the position is Per Diem (offering a fixed rate), salary discussions will occur during the screening process.

Tobacco and Drug Statement

Tobacco use is a well-recognized preventable cause of death in the United States and an important public health issue. Those testing positive are given the opportunity to re-apply in 90 days, if they can truthfully attest that they have not used any nicotine products in the past ninety (90) days and successfully pass follow-up testing. ("Nicotine products" include, but are not limited to: cigarettes, cigars, pipes, chewing tobacco, e-cigarettes, vaping products, hookah, and nicotine replacement products (e.g., nicotine gum, nicotine patches, nicotine lozenges, etc.).
