Job Description​

Saint Luke's Home Care & Hospice is seeking a Transitional Care Nurse to join our team!!

  • Full-time Monday through Friday with flex scheduling to cover one weekend every 1-2 months for telemonitoring
  • Bachelor of Science in Nursing required
  • RN in MO & KS required
  • Certification in Chronic Disease Management, Hospice and Palliative Care required (others will be taken under consideration)
  • 2 years of experience required
  • Clean driving record required
  • Coverage area is Kansas City Metro, may assist in rural areas within Leavenworth, Jackson, Clay, and Platte counties
  • Experience in Home Health, Hospice, Case Management preferred

The Transitional Care Nurse TCN , under the direction of the Director of Operations, serves as a key point of contact for care coordination and transition management for Saint Luke's Home Care and Hospice patients who move from the acute hospital and facility to the home settings and within levels of care when already home. The TCN will support patient self management, advocacy and oversee education and engagement, coaching and counseling of patient and family. Cross setting communication, teamwork and collaboration will be required for patient centered care planning and the nursing process will be utilized for ongoing assessment and outcome setting for each population managed

Certified Hospice and Palliative Nurse - Hospice & Palliative Credentialing Center, Registered Nurse (KS) - National Council of State Boards of Nursing (NURSYS), Registered Nurse (MO) - National Council of State Boards of Nursing (NURSYS)
