Working Hours/Shift:

Monday-Friday, AM shift

What you bring to SBHG (qualifications):

  • Master’s degree in a mental health field
    • Will Consider:
    • Awaiting BBS registration number (time limited)
  • CC II: One (1) year of experience in a mental health field; experience should be with children or adolescents in residential or outpatient settings preferred.
  • CC III - Two (2) years of experience in a mental health field; experience should be with children or adolescents in residential or outpatient settings preferred
    • License or Certification
  • CC II - BBS Associate (AMFT, ASW, APCC) registration, or awaiting BBS registration number.
  • CC III - LMFT, LPCC, or LCSW license in California
  • Valid California Driver's License.

How you will make a difference (job overview):

This position has primary responsibility for Full Service Partnership (FSP) services, including engagement, assessment, mental health services, treatment plan development, care coordination, use of flex funds and case management services to youth and their families. They act as liaison with external agencies and facilitates collaboration with the team.

Division/Program Overview:

  • Supporting individual with mental health, family life, employment, education, living situation, personal effectiveness, and community life functioning.
  • Sessions are in the community, clinic, home and some telehealth.
  • Working with individuals ages 6-15 (Child) and 16-25 (TAY).
  • Utilizing a strength-based approach and working together as a multidisciplinary team.
  • We provide services to individuals and families throughout Santa Clara County.
