Scope of work

The Virtual Nurse is an experienced Registered Nurse (RN) and integrated care team member who leverages technology to collaborate with and support the needs of the bedside RN and the patient/family virtually. Two-way audio/visual technology provides the virtual nurse the ability to assist with admission and discharge activities, patient and family education, real-time patient quality and safety surveillance, proactive rounding, and staff mentoring/education. The purpose of this position is to harness the skills of an experienced RN to support and transfer knowledge to novice nurses within the organization, and to provide high-quality, remote nursing care in accordance with the established policies and procedures of Corewell Health.

  • Works in close collaboration and communication with physicians and other members of the health care team in order to minimize duplication of effort, promote teamwork, and ensure seamless coordination of services within and between teams and across campuses.
  • Assists with patient evaluation and treatment to further their understanding of significant social and emotional factors underlying a patient’s health problem(s). Actively participates in care conferences as appropriate.
  • Mobilizes patient/significant others’ to utilize their own strength to make necessary lifestyle and behavior changes. Acts as a patient advocate to ensure their desires are recognized and followed to the greatest extent possible. Identifies and responds to any situations representing risk to the patient, their family, the organization and/or the community.
  • Partners with community agencies to coordinate care across the continuum to offer options, tools, and resources to allow patients the opportunity to achieve his or her desired goals.

Qualifications

  • Required Master's Degree Social Work
  • Master Social Worker (MSW-Master) - State of Michigan Excludes VNA and Rural Health Clinics (RHC) Upon Hire required Or
  • Master Soc Work LTD - State of Michigan Excludes VNA and Rural Health Clinics (RHC) Upon Hire required
  • Driver's License - State of Michigan Home Care Business Lines Upon Hire required
  • Basic Life Support (BLS) - AHA American Heart Association Rural Health Clinics (RHC) only Upon Hire required Or
  • Basic Life Support (BLS) - ARC American Red Cross Rural Health Clinics (RHC) only Upon Hire required
