Job Summary:

The Patient Navigator serves the entire institution with primary reporting to the Office of the Vice Chancellor. The Patient Navigator works closely with the Office of the Vice Chancellor, External Affairs, Institutional Advancement, and Clinical Leadership Team to support patients, guests, healthcare providers, and staff to ensure a seamless healthcare experience while navigating the UMMC system. The position fosters patient-centered care and delivers respectful, supportive, and collaborative service to patients and guests. The position serves as a referral resource and as a patient advocate. Highly varied working hours.

Education & Experience

Bachelor's degree in applicable job-related field and five (5) years experience in nursing, allied health professions, social work, hospital administration, stakeholder relations, or other relevant field.  BSN preferred.

Certifications, Licenses or Registration Required: RN license is strongly preferred

Demonstrated ability to communicate clearly and persuasively: the ability to plan, manage, and implement patient concierge programs, to project an image of professional competence and credibility, to build trust and keep confidence, and to resolve differences with maturity and judgment. Exhibits communication and problem-solving skills. Demonstrates leadership qualities with an emphasis on teamwork and multidisciplinary relations and interactions. Willingness to travel and give volunteer needs and program demands priority.

Responsibilities

  • Assesses needed resources for patients to facilitate appointments and other encounters with appropriate providers and arrange for pertinent clinical care.  Communicates directly with patients to ensure access to a diverse range of information about relevant disease processes, including materials on potential diagnostic procedures, expected and possible adverse reactions to medication, and health maintenance and disease prevention. Participates in meetings regarding patient care, quality outcomes, and future requirements. Serves as the point of contact concerning inquiries and clinical needs.
  • Coordinates patient and family navigation as a liaison with Service Lines within the Medical Center to strengthen and cultivate relationships. As appropriate and with the patient’s consent, informs UMMC leadership of rounding opportunities. Facilitates the presence and involvement of the patient’s family according to the wishes of the patient and/or the family.
  • Attends to patients, families, and UMMC needs on evenings and weekends as required. Maintains a working knowledge of Epic. Adheres to HIPAA, UMMC privacy and security policies.
