Summary:

Responsible for providing coordination, direct care, health education and support for clients enrolled in assigned program.

Responsibilities:

Coordinates transportation for special activities and events.

Oversees and provides guidance and support to health/wellness coach(s). Ensures all service deliverables are documented in accordance with Program guidelines.

Works closely with medical team and tracks services/appointments to address complex needs of participants.

Collects all pre/post data on grants and grant required outcomes. Prepare report as necessary, including mid-year and end of year reports.

Conducts intakes and assessments and provides wraparound case management utilizing motivational interviewing and the PEARLS (national evidence based Treatment Program) techniques as appropriate.

Complexity of Work: Highly Organized, must be able to work independently and communicate effectively in verbal and written communications. Excellent interpersonal skills, excellent computer skills using Microsoft Excel and Word. Must be able to attend meetings at different locations. Required Work Experience: 2 years of experience working in a social services or primary care setting. Must have knowledge of community resources. Other Information: ** New Hires have 6 months to obtain their CPR CertificationMust have personal vehicle available during work hours as position requires travel throughout Broward CountyMust have valid Florida drivers License and ability to drive CYS Vehicles to transport clients to community events and appointments.
