POSITION PURPOSE:

The Program Coordinator is responsible for developing and facilitating recreational groups and programs which meet the social, emotional, cognitive, and physical needs of the participants in the Dementia Respite Program. The Program Coordinator works as the lead team member providing support to participants which may include assistance with the activities of daily living and will work closely with the Dementia Navigator to ensure a full program capacity.  The Program Coordinator will oversee Volunteers for this program. Program Coordinator will maintain an organized, sanitary program environment and ensure that weekly supplies for programs are maintained and organized. The Program Director will oversee and be present at each weekly program session. This is not a remote position.

ESSENTIAL FUNCTIONS:

  • All essential job responsibilities will be executed in accordance with CGH Health Foundation Administrative policies and compliance/ethics guidelines.

  • Assessing and determining appropriateness of participants into program.

  • Providing specific Dementia related training for Volunteers. Scheduling volunteers to cover 2:1 with participants.

  • Developing groups and program in consideration of the needs, abilities, and interests of participants.  The provision of programs shall offer a variety of formats including small group and one on one, taking into consideration the cognitive abilities of the participants.

  • To research new ideas for programs and activities, taking into consideration the caregiver’s suggestions.

  • Initiating and planning the monthly calendar as well as communicating it effectively with Volunteers to initiate programs and activities.

  • The Program Coordinator will also carry a caseload of no more than 10 participants and maintain a waitlist of future participants.

  • Responsible for all documentation related to those individuals in the program, including all care plans and assessments.

  • All duties must be performed with the highest level of integrity, adhere to CGH Health Foundation Standards of Excellence and support principles of person centeredness. All duties will be executed in accordance with CGH Health Foundation policies and procedures and adhere to all regulatory requirements.

  • Respond to medical emergencies and know when to call 911.

SPECIALIZED KNOW-HOW & REQUIREMENTS:

  • H.S. Diploma or equivalent required.

  • Minimum of 2 years’ experience in working with adults with cognitive impairments, disabilities/functional impairments.

  • Excellent communication skills (written and oral); strong organizational skills.

  • Proficiency in Microsoft Word, Power point and Outlook.

  • The Program Coordinator must demonstrate a positive, professional appearance and attitude, sensitivity to participant’s issues and feelings and excellent communication skills.

Compensation Range

$17.50 - $26.25
