JOB DESCRIPTION:

SUMMARY OF POSITION

This trauma program focuses on providing therapeutic interventions for children and adolescents aged 2-17, promoting personal growth and resiliency through emotional, cognitive, behavioral, and psychological changes. The role involves continuous assessment of clients' needs and functioning, maintenance of comprehensive documentation, and the development and implementation of personalized growth plans tailored to the therapeutic needs of each child or adolescent with a primary behavioral health diagnosis.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Facilitates therapy sessions for individual clients or groups of children and adolescents, addressing trauma-specific needs.
  • Assists clients in developing individualized service plans that are person-centered, family-centered, and service-centered. Monitors client progress in treatment and participates in service plan reviews as assigned, updating plans as necessary and submitting them for medical review when applicable.
  • Conducts intake for new clients entering the program, explaining services, gathering necessary information, ensuring comprehension of all forms, obtaining signatures on relevant contracts and consents, and submitting materials for processing as assigned. Conducts assessments and writes assessment summaries as required.
  • Helps clients connect with internal and external resources outlined in their service plans, coordinating with Integrated Healthcare Services. Maintains appropriate communication with referral sources, providing updates on client progress, behavior, and needs (with appropriate releases), and engaging in ongoing consultations when necessary.
  • Aids clients in developing discharge plans and provides follow-up services as assigned, ensuring continuity of care and support as they transition out of the program.

QUALIFICATIONS

Education Level

Master’s Degree in a human services related field

Years of Experience

Experience providing mental health services preferred.

$48,300.00--$67,700.00
