Scope of work

Provide direct patient care and health support services. Provides assigned patient care under the direction of a RN Case Manager. Assists clients with personal care activities in accordance with the care plan including ADL care, simple wound care, positioning, measurement of vital signs, transfers, ambulation and feeding. Assists with household tasks directly essential to the patient’s health in accordance to the care plan. Observes, records, and reports in a timely fashion, the services provided, patient response to care, and ongoing patient status. Alerts Case Manager and/or designee of potential health problems and changes in the patient's condition. Seeks guidance, direction, and assignment changes from Case Manager responsible for patient's care. Maintains the confidentiality and dignity of patients while helping them to reach their maximum potential in the plan of care.

Qualifications

  • Required High School Diploma or equivalent
  • Experience in residential, rehabilitation or other healthcare setting or experience in education in healthcare field (nursing, healthcare sciences, etc.) Preferred
  • LIC-Driver's License - STATE_MI State of Michigan required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association 90 Days required Or
  • CRT-Basic Life Support (BLS) - ARC American Red Cross 90 Days required
  • CRT-Nursing Assistant - PROMETRIC Prometric preferred
