Minimum Qualifications:
Baccalaureate Degree, ADN, or Diploma. RN Degree Major: Registered Nurse.
Valid Arkansas Registered Nursing License
Minimum of three (3) years RN experience in a clinical environment with current BLS certification.
Preferred Qualifications:
BSN
Care management experience in an Ambulatory setting.
Experience as a participant in continuous quality improvement.
Knowledge or experience in PCMH is desired.
Certification in Case Management (e.g. CCM, ANCC) or obtain within one (1) year of hire date.
Knowledge, Skills, & Abilities (KSAs):
Strong clinical knowledge and experience in managing chronic conditions.
Excellent communication and interpersonal skills to engage patients and interdisciplinary teams.
Proficiency in care coordination and transitional care management.
Ability to develop and implement patient-centered care plans.
Knowledge of value-based care principles and program measures.
Organizational and problem-solving skills to improve care efficiency.
Commitment to promoting patient wellness and self-management.

Key Responsibilities:
Nursing Process:
Triage, assess, plan, implement, and evaluate nursing care and patient education via phone or in-clinic using the care management model.
Develop and manage care plans, assess progress, address barriers, and coordinate care for patients across inpatient and outpatient settings.
Deliver nursing care aligned with UAMS, ANA, AR State Board of Nursing, and specialty organization standards.
Evaluate patient progress toward desired outcomes and ensure care meets regulatory and clinical standards.
Coordinate patient education in collaboration with physician staff.
Education/Evidence-Based Practice (EBP)/Research/Quality Improvement (QI):
Initiate performance improvement and evidence-based practice/research processes.
Evaluate the quality and effectiveness of nursing practices.
Integrate current evidence into nursing practices and promote a positive educational environment for students and staff.
Provide effective patient and family education.
Communication/Collaboration:
Implement interdisciplinary, patient- and family-centered care plans.
Collaborate effectively with nursing staff, interdisciplinary teams, leaders, and stakeholders.
Document care accurately and report care variances through appropriate channels.
Professional Practice:
Engage in unit/department activities and committees, such as the Professional Nursing Organization (PNO).
Commit to personal and professional growth as a nurse.
Evaluate performance against professional practice standards and use feedback to improve.
Maintain clinical competencies specific to the area of practice.
