Registered Nurse (RN) Hospice - Field

Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not cure; on family, not just the patient; and on quality of life, not duration. Cleveland Clinic’s hospice team includes physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers. This team works together to support patients wherever they call home.
A caregiver in this position works three 12-hour shifts from 11:00am - 11:30pm. This position will see patients, mostly, in west Cleveland, Northwest Amherst and Avon.
A caregiver who excels in this role will:
Provide supervision, assessment, planning and direct patient care to hospice patients.
Perform accurate and complete admissions assessments and re-assessments.
Assess patients for safety risks, abuse and neglect.
Assess, document and manage patient’s pain.
Prioritize and document patient problems.
Administer medications and treatments safely and accurately with a focus on patient comfort.
Coordinate care for managing a caseload of patients.
Document changes in patient condition and effectiveness and timeliness of care/interventions.
Evaluate, update and revise plans of care.
Address educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited school of nursing.
Current licensure as a Registered Nurse (RN) in the State of Ohio.
Basic Life Support (BLS) certification from the American Heart Association (AHA).
One year of current clinical experience as an RN.
One year of experience in an acute medical-surgical or critical care setting.
For Hospice field staff only: valid state driver’s license required; per policy 8-175 thereafter.
For Hospice field staff only: proof of automobile insurance with $100,000/$300,000 coverage required; per policy 8-175 thereafter.
Preferred qualifications for the ideal future caregiver include:
Bachelor of Science in Nursing (BSN).
Hospice and Palliative Care Nurse certification.
