JOB DESCRIPTION:

SUMMARY OF POSITION

The Rehabilitation Specialist will provide assistance to clients in gaining skills that are based on their individualized goals. These skills include, but are not limited to, medication management, developing life skills (i.e. cooking, money management, cleaning, parenting, and socializing); pursuing recreational and leisure activities, housing, transportation, legal, medical and financial assistance, and securing appropriate entitlements.  The Rehabilitation Specialist will participate in weekly team meetings to coordinate client care and assist in developing person centered care plans. 

The Rehabilitation Specialist is also required to meet organization stands in timely and accurate documentation, which focuses on the progress and treatment of clients. The Rehabilitation Specialist will work with clients both in the community and in the office, but is primarily a community based position.  Driver’s license required.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Participate in team meetings for the discussion of clinical assessment, distribution of cases and formulation of the treatment plan.
  • Complete clinical documentation that is accurate, meets organizational standards in composition and content, and is adequate for communication about client’s treatment and progress.
  • Meet billable hour productivity standards.
  • Complete ANSA/CANS assessments and reassessments on time.
  • Complete clinical documentation within one business day of the service provided.
  • Complete treatment plans and other clinical service authorizations in a timely manner.

KNOWLEDGE, SKILLS & ABILITIES

QUALIFICATIONS

         

Education Level              

Bachelor’s degree from a four-year college or university is preferred; preferably in psychology, social work, or related field. 

Years of Experience       

Experience working and/or volunteering within the behavioral health, criminal justice, or social services field can be considered in place of degree

Certification/Licensure 

N/A

Pay Range:

$15.75--$22.00
