Job Summary

Under minimal direction, responsible for weekend comprehensive health care services to clients and their families through supervision, scheduling and coordination of services. Applies knowledge of Medicare and State Regulations to the efficient operations of the Agency in addition to the impact of reimbursement from various payor sources.


Essential Functions

  • Assists Liaisons/Care Managers in coordinating and facilitating hospital discharges.
  • Triages and handles all patient calls on Saturday and Sunday during business and after hours.
  • Enters patient information in the computer.
  • Provides complete nursing services and/or care for age related needs of the population(s) served, according to established standards and policies of Home Care.
  • Completes comprehensive OASIS (Outcome Assessment & Information Set) assessments.
  • Initiates the plan of care and revises as necessary.
  • Counsels and instructs the client and family meeting nursing and related needs.
  • Coordinates other interdisciplinary services such as HHA, Therapy, etc.
  • Initiates appropriate preventive and rehabilitative nursing procedures.
  • Prepares and submits accurate and appropriate documentation in a timely manner.
  • Informs the Physician and other staff of changes in the client’s needs.
  • Schedules/assigns patient visits to weekend staff.
  • Distributes and copies weekend and Monday assignment of patients to Managers.
  • Processes Therapy referrals to Therapy Department.

Qualifications
  • Required Associate's Degree in Nursing
  • Preferred Bachelor's Degree BSN
  • 2 years of relevant experience 2-3 years’ experience in Home Care or QA/UR Required
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
