This full-time position is to care for patients in their homes in our East team (serving patients in Eastern Cuyahoga county).

Our Home Care Nursing department is looking to add a Registered Nurse for our eastern counties. This position serves Eastern Cuyahoga, Lake and Geauga counties. As a homecare nurse, you will provide practical support in the home to patients and their families to prevent hospital readmissions. This allows patients to remain safe in their homes for a longer period of time and develop the skills to manage their chronic illness and disease as independently as possible.

As a Homecare Nurse, you will be provided with field training with the support of a preceptor as well as education on the use of homecare specific EMR. We offer tuition reimbursement as well as additional profession development provided through daily huddles, monthly Nursing Townhalls, and quarterly All-staff Townhalls.

Responsibilities:

  • Conducts and submits a complete and accurate Comprehensive Assessment in accordance with OASIS reporting requirements and agency policies and procedures.
  • Identifies immediate care and support needs and obtains orders to address psychosocial and treatment priorities.
  • Makes appropriate admission decisions based on patient eligibility, acuity and safety.
  • Obtains consent for care.
  • Demonstrates knowledge of coverage criteria based on payer source, including: skilled needs, intermittent care, homebound status, and physician ordered care.
  • Other duties as assigned.

Education:

  • Graduate of an accredited school of professional nursing.  BSN Preferred.

  • Knowledge of Home Care and the OASIS data set Competence in, but not limited to the following skills: Infusion therapy and venipuncture Diabetes management, glucometry Wound/ostomy care Urinary catheterization Enteral feeding Oxygen therapy Administration of injectable medication Left Ventricular Assistive Device.

Certifications:

  • Current licensure as a Registered Nurse (RN) in the State.
  • •Current Basic Life Support (BLS) certification through the American Heart Association (AHA).
  • •Valid Driver's license (VDL) upon hire; per policy 8-175 thereafter.
  • •Proof of automobile insurance coverage with $100,000/$3,000,000 coverage, upon hire, per policy 8-175 thereafter.

Complexity of Work:

  • Requires critical thinking skills, decisive judgment and the ability to work autonomously.
  • Must be able to work in a stressful environment and take appropriate action.

Work Experience:

  • Minimum one year current clinical experience as a registered nurse.
  • Minimum one year experience in an acute medical-surgical or home care setting. 
  • Six months of home health care experience preferred.

Physical Requirements:

  • Must be ambulatory and able to drive to patients homes and potential customers, able to climb stairs, pull, push, and facilitate movement of the patient.
  • Heavy Work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.

Personal Protective Equipment:

  • Follows standard precautions using personal protective equipment as required.
