Job Description:

  • Department and location:  Tierney House – Homeless Health Service, St Vincent’s Hospital, Darlinghurst

  • Salary: $66,743.17+ super +salary packaging

About the role

  • Provide 24/7 care, support, and supervision, helping residents access in-reach health services in a safe environment.

  • Work closely with the Homeless Health Service and other partners to improve health outcomes and create pathways out of homelessness.

  • Conduct comprehensive assessments, develop care plans, and refer residents to the right services to meet their individual needs.

  • Promote resident participation and choice, supporting integration into mainstream healthcare and community resources.

  • Maintain accurate documentation and a welcoming, safe, and supportive environment within a non-judgmental, harm-minimisation framework.

About you

  • Has experience supporting residents experiencing homelessness in residential or specialist service settings.

  • Communicates confidently and compassionately, both verbally and in writing, with residents, carers, colleagues, and key stakeholders.

  • Can work independently and collaboratively as part of a dynamic team.

  • Develops and implements care plans that empower residents to engage in daily activities and improve their wellbeing.

  • Is comfortable working with diverse populations, including individuals with mental health, drug and alcohol, and physical health needs, and is computer literate with strong record-keeping skills.

In this role you will be based at St Vincent’s hospital, Darlinghurst.

  • Closely partnered with universities & affiliated research institutes, many of which are located on-campus

  • Centres of excellence include heart & lung transplantation, bone marrow transplantation, cardiology, cancer care, AIDS/HIV, respiratory medicine, mental health, homeless health, drug & alcohol services, and correctional health
