The Continuum of Care Department at St Vincent’s Private Hospital Sydney (SVPHS) provides a coordinated, patient-centred service that supports safe, efficient care across the patient journey, with a primary focus on preadmission assessment and discharge planning.
The department undertakes comprehensive preadmission screening and planning, ensuring that patients are appropriately prepared for admission. This includes early identification of clinical risks, psychosocial needs, and post-acute care requirements, enabling proactive coordination of services and optimisation of patient readiness. The team works closely with medical officers, nursing staff, and allied health professionals to minimise delays, streamline admissions, and support efficient use of hospital resources.
During admission, the department leads multidisciplinary discharge planning, facilitating timely and safe transitions from hospital to home or alternative care settings. This includes coordinating referrals to community services, rehabilitation facilities, residential aged care, and other support services, as well as arranging necessary equipment and follow-up care.
