The Continuum of Care Department at St Vincent’s Private Hospital Sydney (SVPHS) provides a coordinated, patient-centred service that supports safe, efficient care across the patient journey, with a primary focus on preadmission assessment and discharge planning.

The department undertakes comprehensive preadmission screening and planning, ensuring that patients are appropriately prepared for admission. This includes early identification of clinical risks, psychosocial needs, and post-acute care requirements, enabling proactive coordination of services and optimisation of patient readiness. The team works closely with medical officers, nursing staff, and allied health professionals to minimise delays, streamline admissions, and support efficient use of hospital resources.

During admission, the department leads multidisciplinary discharge planning, facilitating timely and safe transitions from hospital to home or alternative care settings. This includes coordinating referrals to community services, rehabilitation facilities, residential aged care, and other support services, as well as arranging necessary equipment and follow-up care.

The Continuum of Care Department is integral to:

• Supporting patient flow and bed management across the hospital
• Reducing length of stay and avoidable readmissions
• Ensuring safe, timely discharge outcomes
• Promoting continuity of care across acute and post-acute settings
• Meeting NSQHS standards and organisational performance targets

Through early intervention, proactive planning, and strong collaboration with internal and external stakeholders, the department contributes to high-quality patient outcomes, operational efficiency, and the delivery of compassionate, person-centred care in line with SVPHS values.

About you 

• You must hold current registration with the Australian Health Practitioner Regulation Agency (AHPRA), with no restrictions on practice, and have a minimum of three (3) years’ post-registration clinical experience in an acute care environment.
• You are an experienced and highly motivated Registered Nurse or Allied Health professional with a strong background in care coordination, preadmission assessment, and discharge planning within an acute healthcare setting. You demonstrate a comprehensive understanding of patient flow, length of stay management, and the importance of timely, safe transitions of care.

You possess:

• Proven experience in multidisciplinary discharge planning and care coordination
• Advanced clinical assessment skills, including the ability to identify psychosocial, functional, and post-acute care needs early in the patient journey
• Sound knowledge of community services, rehabilitation pathways, and aged care systems
• Highly developed time management and organisational skills, with the ability to prioritise competing demands and meet deadlines in a fast-paced environment
• Excellent communication and interpersonal skills, enabling effective collaboration with medical staff, nursing, allied health, patients, families, and external providers
• Excellent computer literacy skills, with demonstrated experience in clinical systems and documentation; experience with electronic patient record (EPR) systems will be highly regarded
• A commitment to patient safety, quality improvement, and continuity of care

You are proactive, adaptable, and patient-focused, with the ability to influence and facilitate timely decision-making to support optimal patient outcomes. You demonstrate a clear understanding of the NSQHS Standards and apply these principles in daily practice.
