Job Summary

The Manager of Quality and Care Transformation is responsible for leading and managing quality improvement and care transformation initiatives. This role involves collaborating with clinical teams, implementing quality improvement projects, and facilitating the delivery of high-value, patient-centered care.

Responsibilities and Essential Functions

Leading and overseeing quality improvement efforts, including performance measurement, data analysis, and reporting.

Collaborating with clinical teams to identify opportunities for quality enhancement.

Driving care transformation initiatives to improve the patient experience, clinical outcomes, and operational efficiency.

Implementing evidence-based practices and innovative care delivery models.

5%

Developing and implementing performance measurement strategies to assess quality, safety, and patient outcomes.

5%

Analyzing data to identify trends and areas for improvement.

5%

Ensuring compliance with healthcare regulations, accreditation standards, and reporting requirements.

5%

Keeping abreast of industry regulations and drive compliance initiatives.

5%

Managing quality improvement projects, including project planning, execution, and evaluation.

5%

Collaborating with cross-functional teams to achieve project goals.

5%

Providing training and education on quality improvement methodologies to clinical and non-clinical staff.

Fostering a culture of continuous quality improvement.

5%

Promoting patient safety and error prevention through evidence-based practices.

5%

Facilitating the reporting and investigation of safety incidents.

5%

Preparing and presenting quality and care transformation reports to hospital leadership and regulatory bodies.
