Position Description:

Administers care coordination and case management services, conducts outreach and education, and facilitates quality initiatives for Healthy Kids Dental (HKD), Healthy Michigan Plan (HMP)/adult Medicaid health plan partners and other tristate Medicaid or Medicare Advantage plans.

Primary Job Responsibilities:

  • Serves as primary point of contact for members and families to facilitate dental care for HKD foster care enrollees, HMP/adult Medicaid plans and other tristate Medicaid or Medicare Advantage plans.
  • Coordinates and facilitates comprehensive care plans for the members’ dental services in HKD and HMP/adult Medicaid, tristate Medicaid plans or Medicare Advantage.
  • Maintains accurate records for member assessments, dental care plan and outcomes in care coordination or case management system as directed for reporting.
  • Provides educational support to members’ and families’ dental health via care coordination or case management, in-person outreach events, virtual outreach events, or other delegated means of member interaction.
  • Creates, updates, and maintains work instructions for case management activities.
  • Establishes partnerships with relevant community organizations to maximize outreach impact by attending state and/or local oral health meetings and conferences and serving on advisory boards.
  • Develops engaging content for outreach strategies to facilitate utilization of the member’s dental benefit.
  • Develops and manages the administration of text and email campaigns for the department, including vendor communications, creation and scheduling of messages to both members and providers, development of reports and evaluating impact of campaigns.
  • Serves as primary point of contact for HMP/adult Medicaid health plans for delegated quality improvement activities, such as member outreach via text, email, newsletters, and in-person meetings, performance improvement projects, value-based payment projects, and care coordination/case management.
  • Identifies priorities, makes recommendations, develops, and executes plans to increase dental benefit utilization.
  • Participates in the development and achievement of short and long-term departmental strategy, goals, and objectives.
  • Assists the QAPI/Population Health Management team with requests, questions, reports and other documents from various internal departments and contacts, including Sales and Account Management, Customer Service, Group Administration, Public Affairs, and Provider Relations.
  • Establishes and maintains relationships with external stakeholders, including MDHHS, other state employees such as Health Liaison Officers and case workers, as well as providers.

Minimum Requirements:

Position requires a bachelor’s degree in dental hygiene, healthcare administration, public health, or a related field and three years of experience as a dental hygienist, and a Registered Dental Hygienist (RDH) license issued by the State of Michigan, Indiana or Ohio or counseling or educating individual patients and/or the general public regarding the importance of good oral hygiene habits, conducting dental research or data collection and analysis, or designing or administering a public health program or dental education program. A community health worker (CHW) certification is preferred. Will accept any suitable combination of education, training, or experience. Ability to speak another language, preferably Spanish or Arabic, is a plus.

Position requires knowledge of dental terminology, procedure codes, and the ability to read and interpret radiographs, electronic images, and photos; knowledge of word processing, spreadsheet, and database applications; strong verbal and written communication skills; strong analytical skills; the ability to represent and present on behalf of the organization internally and externally; the ability to identify, partner with, and persuade key stakeholders and decision makers; the ability to work independently and as part of a team; and the ability to solve problems using independent judgment.

Position also requires the use of a well-maintained vehicle manufactured by Chrysler, Ford, or General Motors while acting as a representative of Delta Dental of Michigan externally. Travel required throughout the tristate area, approximately 50%.
