HOW YOU’LL WORK
- Assume ultimate accountability and leadership for patient assessments and plans of care
- Case manage and provide clinical care under physician direction
- Educate patients and family members, ensuring patient safety at all times
- Work autonomously with strong time‑management skills
- Perform all practices and duties within the scope of practice outlined by the state
MAJOR AREAS OF RESPONSIBILITY
Plan of Care
- Observe and monitor patient condition
- Perform OASIS assessments (training provided if needed)
- Develop individualized care plans and adjust as patient needs change
Minimize Patient Risk
- Administer medications as prescribed
- Prioritize visits for high‑risk patients to help reduce re‑hospitalizations
Supervision
- Oversee and supervise total patient care provided by nurse aides and LPNs
Continuity of Care
- Manage multidisciplinary care as applicable
- Promote continuity through appropriate admissions, transfers, and discharges
Family Educator/Advocate
- Educate patients and families on disease processes
- Utilize teach‑back methods to ensure understanding
Policies and Documentation
- Complete all clinical documentation per agency protocols and Medicare/Federal guidelines
- Collaborate with the interdisciplinary team and physicians to ensure optimal care
Rules and Regulations
- Follow all agency policies, procedures, rules, and regulations
- Communicate schedule or availability changes to schedulers or supervisors
Operations
- Attend required in‑service training and mandatory agency meetings
HARD & SOFT SKILLS
- Compassionate communicator with a positive attitude
- Patience when working with patients, families, physicians, and coworkers
- Strong attention to detail and ability to follow directions
REQUIREMENTS
- Registered Nurse with a current license in the state of employment
- Minimum of one year of RN experience in an acute care setting (home care experience preferred)
- Valid driver’s license and auto insurance in your name
- Ability to meet all physical demands of the position
