Pharmacists are responsible for knowing the purposes, effects, and side effects of various drugs. They should be able to tell from looking at a patient's profile whether or not it is safe to combine new medications with the medications that the patient is already taking. Modern drugs have the potential to significantly improve patient's quality of life, however, they must be taken correctly, with the proper frequency, and in the appropriate dosage.
A caregiver in this position works varying shifts between 7:00am – 8:00pm at various Northeast Ohio Cleveland Clinic locations.
A caregiver who excels in this role will:
Provide pharmacy services and ensure the appropriate selection, dosing, administration and dispensing of medication orders.
Provide safe, efficient, patient-centered, evidence-based medication services.
Verify medication orders and/or dispense medications accurately according to state and federal regulations.
Contribute to medication safety initiatives.
Document services in a complete, accurate and timely manner.
Ensure all pharmacy operations comply with national accreditation standards.
Maintain professional interactions with patients to facilitate successful patient experiences.
Educate and train students, residents, other caregivers, and/or patients.
Be accountable for drug use policy adherence.
Minimum qualifications for the ideal future caregiver include:
PharmD or Bachelor of Science degree in Pharmacy from an accredited college of pharmacy.
Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
For individuals required to administer immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or willing to obtain within 90 days of hire.
For individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA) or willing to obtain within one (1) year of hire.
Preferred qualifications for the ideal future caregiver include:
Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience.
Post-graduate year 2 residency (PGY2) or relevant experience for Clinical Pharmacists hired in a specialized pharmacy practice area (i.e. Critical Care, Oncology).
Board certification.
