We’re seeking a skilled and independent Registered Nurse to join our Trauma Resource team in a newly created role focused on care coordination, discharge planning, and patient education across the trauma continuum. This position is designed to support trauma patients as they transition from inpatient care to recovery, serving as a key connector between the inpatient trauma team and outpatient follow-up. You’ll be part of a highly collaborative, supportive trauma team that values trust, independence, and clear communication. This role offers the chance to help shape a new position within the program while making a meaningful impact on patient outcomes through strong education and care coordination. The ideal candidate will bring several years of trauma, ED, or ICU experience, strong clinical judgment, and a passion for helping patients navigate recovery beyond the hospital setting.
