Patient Navigator – Ambulatory Care Services | Full-Time, Days

Navigating healthcare can be challenging—especially for patients managing chronic conditions, coordinating specialty care, or accessing essential community resources. At NewYork-Presbyterian Queens, our Patient Navigators play a vital role in helping patients successfully navigate the outpatient care journey, ensuring they receive the support, guidance, and resources needed to achieve the best possible health outcomes.

As a Patient Navigator, you will serve as a trusted resource for patients and families, helping them navigate appointments, referrals, insurance requirements, transportation needs, and other barriers that may impact access to care. Working closely with multidisciplinary teams, you will coordinate services, promote continuity of care, and connect patients with community-based resources that support their health and well-being.

The Patient Navigator advocates for patients throughout their ambulatory care experience, helping them understand their care plans, access preventive and specialty services, and overcome obstacles that may delay treatment or follow-up care.

Preferred Criteria

  • Bilingual language skills
  • Master's Degree in a healthcare, public health, social work, or human services-related field

Required Criteria

  • Bachelor's Degree in a healthcare, public health, social work, or human services-related field
  • Experience working in a healthcare or ambulatory care setting and collaborating with interdisciplinary teams
  • Knowledge of HIPAA regulations and patient confidentiality requirements
  • Understanding of population health, care coordination, and community-based resources
  • Demonstrated ability to work independently in a goal-oriented environment while effectively communicating with patients, families, caregivers, providers, and community partners
  • Ability to identify and address barriers to care, including social determinants of health
  • Demonstrated cultural sensitivity and commitment to providing patient-centered care to diverse populations
