Job Description:

Title: Transitions Coordinator Senior Living Division
Status: Exempt Reports to: Transition Coordinator Supervisor
Territory: (North Eastern Ohio Counties) Lorain, Medina, Cuyahoga, Erie, Geauga, Huron, Ashland, Lake, Summit, Portage And all other counties as needed.


PURPOSE
According to prescribed policies and procedures of the organization including all applicable state, federal and accreditation regulations and under the general supervision of the Home Choice Manager is responsible to facilitate the steps needed to assist a consumer from a long term care facility to a home setting in the community.

ESSENTIAL FUNCTIONS
• Serves as a liaison between the health care and housing professionals, including but not limited to the service coordinators, discharge planners, home/community based service representatives. Is knowledgeable on all service lines including adult day health, personal care/private duty, service coordination, skilled care and hospice. Works closely with all managers of all service lines.

• Works closely with individual case manager to assist individual in arranging services based on community resources for the individual’s needs and preferences such as physician, pharmacy, etc.
• Assists the individual with locating housing. Purchases materials and supplies for living.
• Assists on behalf of individual to ensure receipt of entitled benefits.

• Provides quality customer service to all residents to answer questions, address concerns, problem solve, assist, and provide results for the individual with their needs on a daily basis.

NON ESSENTIAL FUNCTIONS
• Assists with the completion of the consumer workbook as needed.

• Assists with linkage to employment options if participant interest is indicated.

• Demonstrates and maintains a positive, professional manner, manages time efficiently and effectively, provides leadership as a role model to promote a team approach, and participates in appropriate problem solving methods.

• With respect to Client Rights, ensures all care is provided with respect and dignity for clients, reports all complaints made by clients and/or families to the appropriate supervisors, reports all allegations of abuse, misappropriation of funds/client property and/or any other corporate compliance items immediately.

• Must adhere to all policies, procedures, terms and conditions set forth in the National Church Residences’ Employee Information Guide (EIG) as well as any facility handbook including but not limited to corporate compliance, drug free workplace, safe work practices, all federal, state, local regulations and laws.

• Performs other duties as assigned.

JOB SPECIFICATION SHEET – Transitions Coordinator

Education: High school diploma or GED required. Bachelor’s Degree in social work or a related field preferred.

Experience: Minimum of 1 years in social service delivery with the elderly and/or family populations. 2-4 years of experience preferred. Demonstrated working knowledge of supportive services and other resources in the area served by the project.

Travel: Frequent, necessary on a daily basis.

Mental: Must have good communication, comprehension, computer and interpersonal skills. Must have the ability to speak, read, write and understand English.

Licensure: Valid driver’s license and able to meet National Church Residences’ motor vehicle policy.
