GENERAL SUMMARY:

Reporting to the Clinical Pharmacy Manager with accountability to the Director of Pharmacy and  Pharmacy Manager, the Clinical  Specialist is responsible for participating in and executing all functions related to clinical pharmacy programs.  Responsible for provide pharmaceutical care which provides safe and effective drug therapy to improve patient outcomes.

II.  REPRESENTATIVE FUNCTIONS: 

Identifies actual and potential medication related problems through review of medication history, patient profile, medical records and interviews with patients and caregivers.  Develops and implements monitoring of treatment plans in consultation with other caregivers to correct problems identified.

Provides detailed therapeutic consultation, drug information, education, and literature reviews to medical, nursing, pharmacy and other allied health care professionals. Initiates patient medication education activities when needed.

Provides education to the pharmacy department staff through quarterly in-services and yearly competency assessments.

Provides education to physicians, nurses, and other health care professionals during daily patient rounding activities as well as through departmental in-services and lectures.  Contributes articles for the monthly Pharmacy Newsletter.

Participates in the activities of various Committees including the Pharmacy and Therapeutics Committee, Medication Safety Committee, CPIC, and other committees related to the specific area and department of specialty. Responsibilities include preparation of formulary reviews and drug monographs, revision and creation of policies and procedures specific to areas of specialty, evaluation of pharmacy and nursing workflows to optimize patient care and evaluation and monitoring of Epic reporting related to areas of specialty.  Participates in and evaluates cost containment measures specifically related to pharmaceutical care as directed by the Clinical Manager.

Participates in the education of rotating Pharmacy Students and/or Residents as assigned preceptors through partnerships with designated Pharmacy Schools

Performs duties as a staff pharmacist in assigned area as necessary; has completed all requirements and training for staff pharmacist position.  Demonstrates a working knowledge of current pharmaceutical literature and practice for all age groups of patients served by the assigned area, and makes sound clinical decision based on this knowledge.

Adheres to all Northwest Community Hospital standards, policies, and procedures.

III.  SPECIAL SKILLS AND ABILITIES REQUIRED:

The communication skills necessary to positively interface with physicians, nurses and other customers.

The interpersonal skills necessary to act as a liaison between the department Director and other departments and areas.

The organizational skills necessary for day-to-day workload management.

The technical skills necessary within the Microsoft Office Suite to professionally prepare the required presentations, in-services and competencies.

IV.  KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

BS Pharm orPharm D degree.

Current license to practice pharmacy in the State of Illinois.

One year post graduate residency preferred or a minimum of three years experience in area of specialty in lieu of residency.

Board Certification required or in process within 24 months of start date.

BLS and ACLS certification within 3 months of hire required.
