JOB DESCRIPTION:

This position has an estimated start date of 11/1. The hours are 8a-4p. Expectations will include collaboration with community partners and resources to build/maintain relationships.

SUMMARY OF POSITION

The Discharge Specialist will work collaboratively with Clients and CSU/WIC teams to develop comprehensive discharge plans for Clients admitted into the CSU.

Supports the Centerstone Mission, Vision and Values in delivering care that changes people’s lives. Communicates clearly and appropriately; provides excellent customer service. Adheres to applicable policies and procedures. Regular attendance and punctuality is required.  

ESSENTIAL DUTIES & RESPONSIBILITIES

Daily meets one-on-one with each Client in CSU/WIC to collaboratively develop a discharge plan that addresses that Client’s presenting problem/primary needs:

  • Mental health: Outpatient appointment scheduling (psychiatric/counseling), safety planning, restricting access to lethal means, connection to follow-up programs
  • Physical health: PCP resources and appointments as needed
  • Substance Use: Relapse prevention plan, treatment resources and referrals as needed
  • Housing: Resources and referral assistance as needed
  • Financial, Legal, Social: Resources and referrals as needed

Participates in daily multi-disciplinary team meetings (CSU/WIC rounds). Maintains and facilitates communication regarding Client’s care with all relevant staff. Displays workforce flexibility including providing direct line services as needed on the unit. Ensures timely and appropriate documentation as required by the Agency. Develops and maintains community and MHC interdepartmental relations as it relates to discharge planning through outreach and prompt follow-up with contacts. Works with Clients during their stay at the CSU/WIC to reduce psychiatric symptoms and facilitate their return to a less restrictive environment

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Perform basic electronic medical record computer skills
  • Detail oriented with effective problem solving skills
  • Knowledge base in community resources
  • Skill in crisis intervention
  • Ability to de-escalate patient/clients using evidence based de-escalation training (e.g. Theapeutic Options) to include safety holds as necessary.

QUALIFICATIONS

         

Education Level              

Bachelor's degree required. Master's degree preferred.

Years of Experience       

Two years in mental health required.
